Membership Application and Release Form

Both new and renewing members must complete and sign a Membership Application and Release Form.

Instructions:

1. Print one or more copies of the membership application and release form.  For family memberships each family member must complete a separate form.
2. Enter information on the form as requested.
3. Make checks payable to: "Rochester Nordic Ski Club"
4. Mail application and check to :
